Service Coordination:
What? Why? How?
Self Paced Training Module
This is a self-paced training module for staff involved in service coordination
activities. The module has been developed to complement agency orientation
and induction programs. It provides you with an introduction to service
coordination, including:

An understanding of the policy context, objectives, principles and
practical approaches of service coordination in Victoria.

An awareness of how service coordination enhances services’ capacity
to respond to consumers’ and carers’ needs.

An understanding of the benefits of service coordination.
-
An overview of the elements of service coordination including Initial
Contact, Initial Needs Identification, Assessment, and Care Planning.

An overview of how referral fits within service coordination.
-
An understanding of the resources available to support service coordination
including the service coordination Tool Templates and Guidelines.
-
A general understanding of Practices, Processes, Protocols and Systems (PPPS) and their role in guiding agency practice.
The training module includes activities and assessment questions that are interactive. The module is divided into a number of parts you can complete separately. This allows you flexibility about when you complete each part and which parts you complete (for example, you may decide that some parts are more relevant to your work role than others; a number of learning pathways are suggested according to work role in the Practice part of the training module).

- CD of the training module for those learners without ready internet
