Presentation 1

Development of Information Management Approach for Eastern Health HARP programs

Eastern Health has commenced a Hospital Admission Risk Program project entitled: Information Management and Business Systems.

The project aims (in the first year) to:

  1. Develop agreed practice standards, business rules and referral protocols for HARP programs and their high-referring primary care partners.
  2. To develop user specifications for technology to support agreed business practices.
    In subsequent years the project aims to pilot and implement a system based on the first years work that can accommodate the many client software programs used by hospital and primary care services.

The project is maintaining a watching brief on the e-Referral developments in the Central and Outer East PCPs. Other pilot projects, such as a client software management system for the Care Coordination unit at the Angliss hospital and regional developments in Community Aged Care Packages' waiting lists, are included in an over-all evaluation of systems and approaches for Eastern Health HARP programs.

Following on from the experiences of the CE & OE PCPs, the project acknowledges the importance of developing and articulating the business rules that will underpin the development of information technology to support practice. The project has commenced by brining together HARP programs and Primary Care agencies to benchmark the Primary Care Partnerships Process, Protocols, Practices and Systems work and Service Coordination Tool Templates against clinical practice.

The presentation will outline the steps involved in this work and the vision for information management and systems development within Eastern Health.

A step or a leap forward into the future

The recently established ACCESS unit was developed to streamline entry into sub-acute services.

A screening and triage process is used to ensure there is adequate two-way communication between ACCESS, internal services and the community.

The challenge has been to develop a single point of contact for all sub-acute services supported by software, adhering to legislative requirements and with the collaboration and co-operation of internal and external stakeholders.

ACCESS provides a comprehensive and evidence based assessment and triage at the point of referral, up to date information and advice and ultimately ensures positive outcomes for consumers and carers.

Primary Care Partnerships is major influence in ensuring the service is integrated with the community sector. Software programs have been introduced to support consistent program management across sub-acute services and provide service co-ordination tools at the point of referral.

Peninsula Health Rehabilitation Aged and Palliative Care Serves are leading the way to ensure there is one strong communication system across the Mornington Peninsula.

Service Co-ordination and e-Referral Project

The aim of the "Service Coordination and E-Referral Project" is to incorporate the PCP service coordination model and implement e-referral processes in the Western HARP Consortium and Melbourne Health HARP initiatives, in order to enhance the care of patients/clients managed by HARP initiatives, by creating a system that assists in integrating, coordinating and improving continuity of care across the acute/primary care interface.

Meeting the challenge of acute to primary care e-Referrals. Melbourne Health's e-Referral system?

The e-Referral Project has provided Melbourne Health with a web based referral, assessment and transfer/waitinglist system to facilitate timeliness, visibility and accountability of intra and intercampus communication. It is now proposed that the system be extended to integrate web-based referrals to community services utilising the SCTT forms via the ConnectingCare.com form templates and regional service directory. The aim is to provide a seamless workflow for clinicians when they are making referrals to internal and external services. It is expected that Melbourne and Western Health HARP Co-ordinators will commence the use of this form of web based community referral early in 2004.
