Clinical Information for General Practitioners: The encrypted emailed discharge summary

The Geelong Hospital has implemented a clinical information system, known as CORDis, using Filemaker Pro database. This clinical information system delivers an inexpensive clinical tool that produces faxed discharge summaries and faxed outpatient letters, faxed EMD attendances, electronic PBS scripts, clinical audit and registrar logs. The general practitioner community in the Barwon Health region has requested the move to emailed discharge summaries. CORDis was readily able to produce a HL7 message for the discharge summary. A review of the currently available enterprise encrypting and emailing systems was undertaken. A pathology system was chosen (Equery), as there was a good track record in delivering electronic results with a minimum of set up time. The emailed discharge summary was easily incorporated into the GP's electronic health record.

GP Links: issues in implementation of e-referral

The GP Links e-referral trial, jointly auspiced by Southcity GP Services and The Alfred, stalled due primarily to implementation issues. These included incompatibilities between clinical application-generated encrypted email and common email clients and business process issues around email management in the average GP practice.

E Referral in the South West

This E referral project involves the development of electronic referral from within a client management system – PJB Software Australia.

Aspects of the project included:

  • The development of ScoTT tools in an electronic form generated from PJB
  • The use of a form of encryption to secure transmission via email ensuring confidentiality
  • The development of agreed referral processes and practice including
    acknowledgement of receipt of referral and referral outcome
  • The testing of both the technological aspects of the system and referral practice prior to recommending a wider application of the system.

The presentation will provide:

  • A background to the project
  • An understanding of the IT context and software use in the region
  • A demonstration of E referral
  • Discussion on developmental, testing and change management issues
  • Comment on outcomes e.g. e referral's impact on practice, its benefits and areas where improvement is needed
  • Future developments

Connecting with GPs – Implementation of Electronic Discharge Summaries

Peninsula Health has implemented an electronic patient information system, which includes e-prescribing and e-ordering applications, patient status notification to GPs and electronic discharge summary application.

The project aims to improve the quality of patient information, reduce clinical risk and improve communication with local GPs.

The implementation of electronic discharge summaries commenced in January 2003. The percentage of summaries sent electronically has increased from 10% in the first month of implementation to over 70% in September 2003. GPs are also more positive about the timeliness and quality of discharge summaries received.

A number of technical and cultural barriers have impeded the implementation and these will be discussed.
