Service Coordination Electronic Referral System: Do the BITS & BATS add up?
The Outer East Health & Community Support Alliance (hereafter the Alliance) has continued to progress the work undertaken by the City of Maroondah Best Practice Working Group, titled the Maroondah Information System Development Project. The objective of the project was to develop and deliver an electronic client referral system for eight participating agencies in the City of Maroondah. One of the features developed in the model was an "agency agreement". Participating agencies agreed to receive and act on client information pertaining to a defined service, providing that certain conditions were met.
The outcome of this project was the successful piloting of an E-referral system and the development of an IMIT platform for further refinement and application in the Outer East Primary Care Partnership and other human services environments.
The introduction of the BATS strategy provided a strong framework for defining practices, processes, protocols and systems for the development of local Service Coordination strategies. The ongoing development and implementation of the IMIT platform provided a focus for the development of local strategies, which in turn informed IT development. The close relationship that developed between developers and service providers has led to a strong ongoing partnership focused on improving service coordination.
The Alliance has commenced the second phase of this project that sees a re-design of the E-referral system, incorporating a number of enhancements and the rolling out of the enhanced system across the whole of the Alliance.
It is the shared learning, business processes and re-design that will form the bases of the "BITS & BATS" presentation. Strengths, enhancements and challenges will be explored as a part of our learning and consideration for the way forward.

Establishing the online Service Directory & e-Referral across Western Metropolitan Region agencies
To support Service Coordination implementation through improving access to information about services available, and client support and referral through an electronic comprehensive service information facility. This project includes development and implementation of:
- A local Electronic Service Directory to support service coordination
- An Electronic Referral System between designated PCP agencies and other service provider using PKI secured email and SCTT online forms or attachments
building on work previously undertaken on privacy and information security and inter-agency referral protocol development.
Implementation is now commencing with GP divisions and practices, sub-Acute services and Melbourne and Western health services to support common practice and use of standardised referral forms to community settings.

Electronic Referral, workplace issues and communication
A joint presentation will be given by Ann-Marie Deeker (Care Coordination Unit Box Hill Hospital) and Brett Wake (CE PCP).
Brett will give a brief introduction and provide the CE PCP context. Ann-Marie will talk about the issues for practitioners and the experience of the Care Coordination Unit in Box Hill Hospital as participants in the trail and ongoing pilot phase.
The presentation will focus on the implementation issues for practitioners and the ways that barriers to participation were over-come during the trial.
The presentation will include issues of communication between practitioners and the emerging possibilities for care planning and 'shared care plans' as they exist within the Central East catchment. Emerging possibilities for a regional approach to electronic referral in the Eastern region will be outlined.
In particular the presentation will include feedback on the impact of this development on an Eastern Health HARP project – Community Hospital Integrated Response Program (CHIRP).
The presentation will cover the implementation of an e-referral trail and the business and referral protocols that were incorporated into the trial software. The incorporation of protocol prompts and feedback loops into the system, the ability to provide a comprehensive audit trail and security issues will be covered.
