Overview

Primary Care Partnerships (PCPs) are required to report against their e-referral activity using the PCP e-referral reporting tool. Data reported through the reporting tool includes monthly e-referral volumes for the period July 1 2009 to June 30 2010 and e-referral traffic between services for the period June 1 2010 to June 30 2010. The collected data will be aggregated and used to further progress the activities of PCPs. The collected data will also be used to develop an 2009-10 PCP e-referral report similar to the e-Referral capacity building through Primary Care Partnerships 2006-07 - Summary report. The reporting tool should be completed, saved and emailed to pcp@health.vic.gov.au  by 30 September 2010.
