Service coordination

Service coordination reform aims to place consumers at the centre of service delivery - ensuring that they have access to the services they need, opportunities for early intervention and health promotion and improved health outcomes. Service coordination is facilitated by Primary Care Partnerships where agencies come together to agree on how they will coordinate their services so that consumers experience a health system that works together.

Information in this section of the site includes:

  • Privacy resources
    Resources which aim to assist practitioners in their discussions with consumers regarding how their information will be dealt with.
  • Implementation of service coordination
    Implementation of service coordination has meant a substantial level of activity in the sector. Many PCPs have volunteered to share the resources they have developed and these have been posted on this site. Also available at this site is the project report prepared by KPMG looking at the impact of service coordination on 8 community health and local government Home and Community Care providers.

  • Workforce development
    Service coordination requires a change to the way agencies do business. Workforce development resources are available to support this change.
