Community health plans31 Primary Care Partnerships, comprising over 800 services, submitted their 2004-2006 Community Health Plans to the Department of Human Services on 30 June 2004. The 2004-2006 Community Health Plans describe the changes that are being made to the primary health care system in Victoria as a result of the Primary Care Partnership Strategy. The Primary Care Partnerships are beginning to make a significant difference to the sector, these changes will eventually lead to significant improvements in the health and well being of the Victorian population. The following Community health plans also include the Integrated Health Promotion Plans for each PCP. If you already know which PCP you belong to you can view the Community Health Plans for your PCP by going to the specific PCP webpage.
