Overview

These guidelines have been developed by the Department of Human Services (DHS) to support Primary Care Partnerships (PCP) in the development of a three year strategic plan for 2009-2012 and in the implementation of new PCP planning and reporting requirements. The guidelines form part of the planning and reporting requirements for the funded activity 28087 detailed in the Primary Health Branch, Funded Organisation Requirements 2009-2012.
