Information for service providers
Community Health Services (CHSs) are agencies that receive Community Health Program funding from the Department of Health. Typically these agencies are registered community health services, public hospital and health services (including small rural health services). There are approximately 100 CHSs in Victoria operating from approximately 350 sites.
CHSs are active participants in and contributors to their local communities. This strong connection enables CHSs to develop models of care that are responsive to their consumers and reflect the diverse underlying determinants of health. In this way, CHSs combine the social model of health with clinical care to maximise outcomes for their consumers.
