The Improving Chronic Care (Wagner) Model

The Improving Chronic Care (Wagner) Model provides a framework for improvement that identifies the elements of a service system that are essential in responding to the needs of clients with chronic and complex care needs. This site provides information concerning the six elements of the model that encourage high-quality chronic disease management across a variety of chronic illnesses, health care settings, and target populations.

Wagner Chronic Care Model Toolkit
This online toolkit provides step-by-step descriptions of the specific changes involved in Chronic Care Model implementation, including more than 60 tools created by leading organisations for quality improvement initiatives, strategies to address financial and operational barriers to quality improvement and stories from organisations that have undertaken successful quality improvements and service system developments to improve chronic care.

Assessment of Chronic Illness Care (ACIC)
A simple, self assessment tool used to assess an organization's current levels of care with respect to the six components of the Chronic Care Model (community resources, health organization, self-management support, delivery system design, decision support, and clinical information systems); developed by Improving Chronic Illness Care (Seattle, Washington, USA)

Better Healthcare in Gippsland Resource Kit
