The Plan, Do, Study, Act (PDSA) Model for Improvement Project

The Plan, Do, Study, Act (PDSA) Model for Improvement project was a 12 month project (concluding in November 2011) which aimed to improve care for clients with chronic and complex conditions through improved systems of practice. The project is a component of the Chronic Disease Incentive and Innovations project.

It involved participants undertaking small rapid cycles of quality improvement using the PDSA Model for Improvement. A key feature of this approach is the use of data to measure change and effect.

The project had three components:

  1. Approximately 50 agencies were funded to undertake quality improvement activities using the PDSA methodology. They were supported through training and telephone support.
  2. Local divisions of general practice have been funded to assist the quality improvement through their links to general practice, and draw on their experiences of the PDSA methodology used in the National Primary Care Collaboratives project.
  3. Primary Care Partnership (PCP) staff were also crucial partners to assist agencies through the process. They were supported through training and telephone support.

This project supports the Department of Health policies regarding the management of chronic and complex clients1, including the Primary Care Partnership (PCP) strategy to improve service co-ordination practice and integrated chronic disease management.
