Patient Flow Collaborative II - Outpatients

Congratulations to the PFC II – Outpatient teams

The PFC II – Outpatients celebrated its conclusion at the MCG on the 8 June 2007. Many teams achieved significant results in patient flow, measured by improvements in waiting times, better discharge rates and reduced numbers in patients who fail to attend. Many teams reported a change for the better in staff surveys and their working environment.

The community health teams have forged strong relationships with health service organisations to provide better services for local communities.

Many teams have achieved results not thought possible 12 months ago. For further information about the final show case see Events.

The final report outlines the many achievements of the Collaborative and can be downloaded from Communications.

About PFC II

The PFC II – Outpatients project is a unique opportunity to tackle patient flow constraints across Victoria’s outpatient departments.

The project provides tools, resources and programs that will help Victorian health organisations to make lasting improvements.

The key project features are:

  • use of the collaborative methodology to foster service improvement and better patient experiences
  • preparing program measures from a minimum data set, showing statewide improvements
  • improvement work based on the patient journey, including the community-acute interface
  • a whole-of-Department of Human Services approach, involving a representative steering group.
