Aim

To effectively manage patients who require elective/emergency surgery

Change Concepts

  • Review current waiting list policy and provide training in policy with reviews
  • Establish care plan for patients who are pre-op assessed not fit for surgery and do not add to waiting list
  • Map process from clinical discussion to schedule operations
  • Use primary target lists - set target number of patients to be treated by set date
  • Develop electronic waiting list management module that shows patient waiting times by consultant
  • Apply capacity and demand to current waiting lists and establish weekly reviews of demand
  • Audit process
  • Model demand and conversion from out patient clinics

Key Change Principles

  1. Manage the waiting list as a 'live' list containing patients who are fit for surgery
  2. Validate waiting list as a way to queue manage patient expectations
  3. Develop 'Buffer' of pre-assessed patients who are willing to be called at last minute if capacity becomes available.

  • Patient Journey Time on Waiting List (SPC chart)
  • Patient Waiting Times for Admitted Patients from Waiting List (Pareto chart)
  • Hospital Initiated Postponements per 100 Admissions (Line chart)

Case Study Measures

  • Number of patients waiting broken into monthly/quarterly cohorts.

HDM Supplementary Data

  • Hospital performance
    • Peer group performance charts
    • Bar charts showing how hospitals stack up against each other and time series charts showing how they have performed within their peer groups in recent years.
    • Waiting times of admitted patients by hospital. Useful source of procedural level data.
  • Hospital Exception Reports - From June 2003 onwards
    • Provides useful information on where individual hospitals are not meeting demand
  • Control Report and Supplementary Report
