Better clinics, improving the patient journey toolkit

St. Vincent’s Hospital Melbourne is committed to the delivery of a co-ordinated, flexible and high quality ambulatory care service, responsive to individual needs. A key objective of the ambulatory service strategy was the organisational redesign of staff and processes within St. Vincent’s (Outpatient) Clinics (the clinics).

The ‘Better Clinics’ Project was the vehicle to drive this change. This toolkit outlines the key elements of the Better Clinics project.

The broad goals of the project were to:

1. Improve the effectiveness and efficiency of the service offered to patients.
2. Improve the referral process for General Practitioners and other referring agencies.
3. Enhance the roles of medical, nursing and clerical staff working within the clinic.

• Divisional Managers
• Hospital Demand Management Co-ordinators
