PFC 2005 - About the collaborative

The Patient Flow Collaborative is the first Victorian initiative to enable a hospital-wide review of patient services, in order to identify and improve Victorian patient Care.

All major metropolitan and large rural health services are participating in the 18 month program.

The Clinical Innovation Agency ,Office of the Chief Clinical Advisor is managing the program, with funding from Hospital Demand Management, Department of Human Services.

The initiative follows successful Collaboratives in:

  • National Medication Safety Breakthrough Collaborative (2003-05)
  • Blood Matters (2003-04)
  • Acute to Subacute Patient Flow (2003-03)
  • Adult Intensive Care (2002-03)
  • Emergency Department Care (2000-01).
