Promoting Partnerships in Palliative Care Services in Victoria Project

This Commonwealth funded project aimed to find out what creates continuity of care for people with a life threatening illness and provide specific recommendations about how to strengthen the hospital-community interface in palliative care.

PPR Consulting were appointed to conduct the project and a working party drawn from inpatient and community based palliative care services provided additional information and advice.

The methodology for the project included a review of the literature and consultations with palliative care providers. Many thanks to the palliative care and related services who took part in the consultations for this project.
