Post Acute Care Service Models EvaluationIn April 2007, consultants were engaged to undertake a review of PAC Service models. The key areas identified for review were:
Over 130 PAC employees and other key stakeholders took part in in-depth interviews to explore these issues. Over 540 stakeholders responded to a survey to further explore the issues identified during the indepth interview phase of the review. Some of the key findings from this review were:
