Summary BookletImproving care for older people – a policy for Health Services outlines how a ‘person-centred’ approach to the care of older Victorians by Health Services can benefit the whole community. ‘Health Services’ include the acute and sub-acute campuses of a Health Service, as well as the additional programs that a Health Service provides in the community. This summary booklet outlines ways in which everyone can move towards
more person-centred care of older people. It contains practical tips
on getting started and examples of ‘policy in action’. This
is just a glimpse of the issues covered in the full
policy paper. |
