Nasal CPAP

Nasal continuous positive airway pressure (NCPAP) is an effective treatment for respiratory distress syndrome. Whilst there is extensive experience in the use of NCPAP in tertiary units, the use of NCPAP in Level 2 Victorian nurseries as a definitive treatment to reduce the need for transfer to a tertiary centre is uncommon and heterogeneous.

In recognition that there is considerable variation in practice in the use of NCPAP in Level 2 hospitals with special care nurseries, a multidisciplinary group of the Neonatal Services Advisory Committee developed guidelines on Administration of nasal CPAP in non-tertiary Level 2 nurseries. The guidelines were developed to assist Level 2 units wishing to utilise this therapy to do so in a safe, effective and efficient manner. Their purpose is to provide best practice guidelines to optimise the care provided by those Level 2 nurseries that do wish to deliver this procedure.

The guidelines cover four main areas:

  1. a clinical protocol that encompasses indications and contraindications, technique of administration, assessment of success/failure, weaning and discontinuation, complications and monitoring requirements
  2. medical resources
  3. nursing resources
  4. equipment.

The guidelines emphasise that:

  • there needs to be clear communication links with tertiary units. Each baby should be discussed with a neonatologist prior to commencement of NCPAP and on a daily basis thereafter.
  • hospitals should have a clear policy on the use of NCPAP. This should include provision for ongoing professional development for staff in relation to prolonged administration of respiratory support. In developing this policy and procedures document, input from hospital management as well as medical and nursing staff is required.
