(External link) The Symposium publication 'Shared Care' - Proceedings
of the 1997 Geigy Psychiatric Symposium, 1999. Novartis, Sydney has
further information on CLIPP, (reference 5 above) and another paper which
sets the CLIPP model in a framework for liaison with General Practice
more widely:

Conditions
The desire of the authors and of the Department of Human Services Victoria
in supporting this publication is to see the procedures and structures
presented adapted and developed for use in other settings. Also we share
a wish to gather information on the uptake of the model or elements of
it. Readers who wish to adopt or adapt elements of this publication should
apply to the developers at email clipp@medicine.unimelb.edu.au.
They will receive a brief application form. This will detail the requirement
of a license for use and adaptation of the materials.
Those registered will be required to undertake the completion, each year
for the three years after registration, of a single sheet form to give
some feedback regarding the use of the materials. Following agreement
with the above conditions, applicants will be granted a license to freely
adapt and develop their own materials based on those in this manual including
directly using copy extracted from the electronic version if this will
be helpful. They will also have access to additional versions of the manual,
specifically:
- A printable version of the Acrobat reader file
- MS Word templates of the forms
Due acknowledgement of source in any publications will be required. Usage
of these materials other than as specified above may constitute a breach
of copyright.Requests for licenses to make use of the materials or for further
information on the model can be directed to clipp@medicine.unimelb.edu.au
