How Case Management Can Help You <

Carrying Out the Individual Service Plan

The case manager will work with you to look at:

  • Your current situation-that is, your strengths, your resources and your problems.
  • What goals are needed to improve the situation.
  • What strategies are going to be used to achieve these goals.
  • Who will be involved in carrying the strategies out.
  • A date for the review of the ISP.

Your case manager will usually be in contact with other professionals and services to make sure your needs and goals are looked after quickly and appropriately.

Remember, it is the case manager's responsibility to make sure the ISP is carried out in the best possible way.

As previously mentioned, your ISP will usually be reviewed at least every six months. The review will usually include you, your case manager, your family, carer or advocate, if you agree to their involvement.
