Linking Municipal Public Health Plans and Community Health Plans

Municipal Public Health Plans and Community Health Plans are complementary. They share a common aim to focus planning on local areas and empower local communities to work together on key health and well being issues. It is important to avoid duplication of consultation and data collection at a local level.

This Information Resource, available below, has been prepared jointly by the Public Health Group and Community Health Branch. It:

• describes Municipal Public Health Plans and Community Health Plans, explaining how they relate to each other; and

• contains a number of case studies, providing examples of good work and the benefits to communities of collaboration in planning efforts.

There are more examples of innovative use of Community Health Plans and Municipal Public Health Plans. If you wish to submit a further case study, please contact Kristina Basile or Holly Piontek-Walker. These will be posted on the Primary Health Knowledge Base and the Local Government Planning for Health and Wellbeing website.
