Make a complaint
There are several ways for you to send us your complaint.
You can now make an online complaint. This is the quickest and easiest way to reach us.
To discuss your complaint, get advice on taking your complaint directly to a provider, or for assistance filling out your complaint form call us on 1300 582 113 between 10am and 4pm, Monday to Friday.
When you contact us the Assessment Officer will:
