Patient & Client Management Systems
Overview
Patient management and client management systems are used to support all functions associated with the administration and management of patients and clients. As all services in a hospital or community health service are reliant on correctly identifying individuals, this function is absolutely critical to the delivery of services.
PCMS
HealthSMART’s integrated patient and client management system (PCMS) i.Patient Manager supports the administration and management of patients and clients throughout their care in hospitals and/or community health settings. It has two major components, an acute module and the community module. The major advantage of using the integrated system across individual agencies is that it supports an integrated workflow across these agencies, supporting a continuity of patient care.
PCMS is being rolled out to metropolitan public health services and rural health alliances. Ten agencies are scheduled to be live with PCMS by end 2009 with Eastern Health to go-live in 2011.
CMS
HealthSMART’s client management system (CMS) TrakCare supports the administration and management of clients at standalone community health centres in metropolitan areas.
22 community health services are scheduled to be live with CMS by end 2009.
Key benefits of PCMS and CMS:
- Staff now have instant access to patient records.
- Patient records are more accurate because they can be updated instantly on or offsite.
- Staff are better informed about a patient’s medical history with automatic alerts, for example, notifying of a patient’s allergy to medication or history of chronic illness.
- Updating to TrakCARE and i.Patient Manager has allowed health agencies to ‘cleanse’ their patient/client records – picking up whether a record is incomplete or if there are discrepancies across records.
- Care is better coordinated across hospital departments and between services.
