Terms & conditions
All entries must adhere to the following conditions of entry or risk being declared ineligible:
1. Entries will be accepted from Victorian government-funded healthcare providers and organisations delivering public health research or programs. Organisations eligible to apply include, but not limited to, hospitals, community health services, mental health services, aged care services, integrated care centres, ambulance services, and volunteer, charity and non government organisations, organisations delivering public health research and initiatives. Organisations which have worked collaboratively with another organisation/s can jointly submit for an Award.
Entrants in the Premier’s Health Service of the Year Award must be included in the list of eligible organisations and have registered their intention to enter using the online registration form (which will be available from 18 July 2011) by 1 August 2011.
2. Entries must be submitted in accordance with the important entry information set out on the Awards website and comply with the following:
(a) All entries are to be submitted electronically using the correct entry form sent by email from the online submission website by 24 August 2011. Late entries cannot be accepted.
(b) The entrant’s chief executive or equivalent for public health endeavours must endorse each entry prior to electronic submission.
(c) Entries in the 2011 Awards can be lodged for a single Award only.
(d) Previous winners (including gold and silver Awardees) or past winning initiatives cannot re-enter for a Victorian Public Healthcare Award.
3. The Department of Health reserves the right to invite entries.
4. Before entries are accessed by the judges, they will be vetted by the Department of Health to ensure the entry complies with relevant regulations and standards.
5. The judges reserve the right not to confer an Award in any given division. The judges also reserve the right to declare joint winners in any given Award.
6. The decision on winners and finalists in each category, including the Minister’s Awards and Premier’s Awards, is final and no correspondence will be entered into.
7. No hard copy supporting information, such as videos, CDs or posters, will be accepted, unless by prior arrangement.
8. Copyright. All entries submitted to the 2011 Victorian Public Healthcare Awards assign copyright ownership of the entry to the Victorian Department of Health. The work may be published, used for promotional material, media interviews or as models to support policy development or programs.
9. It is intended and agreed that the conduct of the Awards shall not give rise to any legal obligations or duties, valid or enforceable in any way against the Department of Health.
