Software Application Form and Download

(1) Hospital completes on-line PICQ Software Application Form to:

(a) provide required contact details,
(b) automatically request software password,
(c) acknowledge the terms and conditions of the licence.

(2) DHS will email the hospital with the password once the application form has been received.

(3) Hospital downloads software from password protected web page.

If you experience difficulties in downloading this software or should you have a slow connection, please contact HDSS at HDSS.HelpDesk@dhs.vic.gov.au and DHS will send you a CD.

After your Application Form has been processed and you have received your logon & password, click here to access the PICQ Software

(1) Access University of Sydney's licence agreement and application form:

Application Form (PDF File 52 KB) (Note: Includes the License Agreement and Application Form)

The application forms are necessary to:

(1) provide required contact details,

(2) Complete the form and forward to:
