Nutritional Risk - Training ManualPage content: Summary | Download documentsSummaryThe purpose of this training program is to provide community services in aged and disability care with a Nutritional Risk Screening and Monitoring Tool and resources to support intervention. Increased awareness of nutrition-related problems and timely intervention can prevent deterioration in quality of life and ill health, and premature loss of independence. This training program demonstrates and advocates for the introduction of nutritional risk screening and monitoring to the assessment process with home-based people. It also gives many practical suggestions about solving client problems and information on where further assistance may be sought for them. The materials in the Training Manual and the materials in its companion, the Resource Manual can be included in general assessment and intervention processes for independently living frail older people, younger adults with disabilities, and financially disadvantaged adults living in alternative accommodation.
