Access Points Mapping ProjectPage content: Overview | Appendix OverviewThis project is part of the National Community Care Access Points initiative, whose aim is to simplify entry to the community care system. Its findings will contribute to improved navigation for frail older people, younger people with disabilities, carers, families, friends and service providers. Client pathways and entry points were explored by analysing data on people who received a service from the Home and Community Care (HACC) Program in Victoria in 2005-06. According to the HACC minimum data set (MDS), just over 229,000 people received a HACC service in that year. No comparable data were available on people who had only enquired about a service.
Appendix: Tables on community care services and programs in VictoriaThe accompanying Excel tables provide quick at-a-glance information on the services and programs that were examined in the Access Points project. It is not exhaustive list and may not be complete for some programs due to data availability. Information regarding Community Aged Care Package (CACP) and Extended Aged Care at Home program (EACH) was obtained from the Department of Health and Ageing Website on 23 August 2007. Source: http://www.health.gov.au/internet/wcms/publishing.nsf/Content/ageing-rescare-servlist-download.htm The volume of transactions (represented as number of clients) and client mix (number of clients with culturally and linguistically diverse background and indigenous clients) are estimates based on HACC MDS 2005-06. Client estimates for Aged Care Assessment Service (ACAS) are based on ACAS MDS 2005-06. The counting rules and methodology used in this project are different from other published reports. For instance, the estimated number of HACC clients is based on service outlets in each region, which are then aggregated at agency level. Hence, it serves as an indication of volume and may not tally with other reports. Caution is therefore needed in applying the data for other purposes.
