ASM PREPARE and ASM PREPARE - Express

An overview

ASM PREPARE and ASM PREPARE - Express are practice review and planning tools developed to assist HACC funded agencies:

  • Develop a structured view of their current strengths and areas for improvement in delivering an ASM approach
  • Identify current practices that are already consistent with an Active Service Model philosophy of care
  • Identify changes in structure and practice required to move to an Active Service approach and
  • Prioritise and plan their implementation strategies for their initial agency implementation plan due on 30 September 2010.

ASM PREPARE was the first tool to be developed. An outcome of the piloting process was identifying the need for a more streamlined tool as an option for smaller agencies, and those funded for only one or two HACC service types. This led to the development of ASM PREPARE - Express.

The key difference between ASM PREPARE and ASM PREPARE - Express is how they deal with the point of service commencement. In the ASM PREPARE tool, there is an in-depth consideration of the steps of Initial Contact and Initial Needs Identification, Assessment, Care Planning and Review. This reflects the roles of providers of multiple services, and those with a strong focus on assessment services, such as HACC Assessment Services. ASM PREPARE – Express rolls up Initial Contact and Initial Needs Identification, Assessment, Care Planning and Review into one combined section called Service Commencement. 

ASM PREPARE

ASM PREPARE is structured into three parts:

Section 1: Orientation to ASM PREPARE

This section gives an orientation to ASM PREPARE.  It is recommended reading for all staff participating in practice review.

Section 2: Managing the Review Process

This section is about managing the review process. It is recommended reading for the person or group responsible for managing the practice review process.

The Action Plan section of ASM PREPARE (page 15) is available as a separate word file to allow organisations to document and save an electronic version of their action plans.

Section 3: Practice Review and Planning Tools

This section contains the Practice Review and Planning Tools for each practice review area that is:

  • Initial contact and needs identification
  • Assessment
  • Care Planning and Review
  • Service Delivery
  • Service Closure and/or Transition.

ASM PREPARE – Express

ASM PREPARE – Express follows a similar structure to ASM PREPARE but due to its condensed size it has been combined into one document.
