ASM PREPARE was the first tool to be developed. An outcome of the piloting process was identifying the need for a more streamlined tool as an option for smaller agencies, and those funded for only one or two HACC service types. This led to the development of ASM PREPARE - Express. The key difference between ASM PREPARE and ASM PREPARE - Express is how they deal with the point of service commencement. In the ASM PREPARE tool, there is an in-depth consideration of the steps of Initial Contact and Initial Needs Identification, Assessment, Care Planning and Review. This reflects the roles of providers of multiple services, and those with a strong focus on assessment services, such as HACC Assessment Services. ASM PREPARE – Express rolls up Initial Contact and Initial Needs Identification, Assessment, Care Planning and Review into one combined section called Service Commencement. ASM PREPAREASM PREPARE is structured into three parts: Section 1: Orientation to ASM PREPAREThis section gives an orientation to ASM PREPARE. It is recommended reading for all staff participating in practice review.
Section 2: Managing the Review ProcessThis section is about managing the review process. It is recommended reading for the person or group responsible for managing the practice review process.
The Action Plan section of ASM PREPARE (page 15) is available as a separate word file to allow organisations to document and save an electronic version of their action plans.
Section 3: Practice Review and Planning ToolsThis section contains the Practice Review and Planning Tools for each practice review area that is:
Section 3 of ASM PREPARE is also available as a separate word file to allow organisations to document the findings of their review process in electronic form. ASM PREPARE – ExpressASM PREPARE – Express follows a similar structure to ASM PREPARE but due to its condensed size it has been combined into one document.
The Action Plan section of ASM PREPARE- Express ( Section 2: page 12) is available as a separate word file to allow organisations to document and save an electronic version of their action plans.
Section 3 of ASM PREPARE - Express, which contains the Practice Review and Planning Tools, is also available as a separate word file to allow organisations to document the findings of their review process in electronic form.
