Viewing MDS reports on the Funded Agency Channel (FAC)

Overview

HACC Service Providers can access their quarterly HACC MDS (Minimum Data Set) feedback reports on the Internet by logging onto the Department's Funded Agency Channel (FAC) website.

The HACC MDS feedback reports are intended for Victorian HACC service providers.

Note: the material on FAC will be updated daily during the transmission period. This gives you an opportunity to correct and re-send any files before the end of the collection period.

How do I access the HACC MDS reports?

To access the HACC Client Profile and HACC Service Provision reports:

  1. log in to FAC
  2. click on the My Agency tab
  3. scroll down to the Data and Performance section
  4. click on the HACC Reports link
  5. scroll to the report you want and click on that link

Note: You must be registered to FAC access these reports. To register go to the FAC home page and click on "Register" (next to the log in box).
