Who uses the Common Application Form?

This form is currently in use at the following institutions:

  • Family Planning Vic Inc
  • Freemasons Hospital
  • Goulburn Valley Health
  • International Diabetes Institute
  • Latrobe Regional Hospital
  • Melbourne Health
  • Mercy Health and Aged Care
  • Monash Surgical Private Hospital
  • Northeast Health Wangaratta

The following institutions will accept applications on the form when researchers are applying to more than one human research ethics committee:

  • RMIT University
  • The University of Melbourne
  • West Moreton Health Service District QLD

Researchers should check with the HREC of the relevant institution for any local requirements or conditions for the use of the form. Other institutions not listed are welcome to use the form, but we request that you contact the Executive Officer of the Department of Health HREC prior to using it.
