How to use the formThe form consists of a number of modules, all of which have been revised to conform to the National Statement on Ethical Conduct in Research (2007). All applicants must complete:
Applicants undertaking specific types of research must also complete the additional module/s relevant to their research. Each module includes guidelines for answering the questions. We recommend that applicants read the guidelines before completing the form. To submit an application, download the appropriate modules, complete them (using Word), and then consult Applying to Other HRECs for information on how to submit. Download the form modulesAll modules are available in Microsoft Word 6/95 format. To save a file to your local directory, click on the relevant link; if the file opens without a 'save' prompt, use the Save As option from the File menu.
