Continuity of care (post-withdrawal linkage support)

Continuity of Care seeks to ensure that clients get continuous care when moving from one service type to another. This has particularly important with complex care clients with behavioural disorders who are often involved with multiple agencies including Alcohol and Drugs, Mental Health, Juvenile Justice and Housing.

Target group

Clients who have undergone a drug withdrawal program and who need to be linked to ongoing support to maintain therapeutic change.

Key service requirements

  • To ensure that each client who completes a withdrawal program is linked to post-withdrawal support at a specialist drug treatment counselling service or to a residential rehabilitation service.

  • To meet with the client and the withdrawal program during the withdrawal to assist with discharge planning and linking with post-withdrawal support.

  • To establish arrangements with specialist drug treatment counselling services to ensure ease of transition for the client between programs.

  • To provide secondary consultation and training to alcohol and drug workers.

  • To provide case planning as appropriate in conjunction with both the withdrawal and counselling services.

  • To facilitate access to other immediate post-withdrawal services as appropriate.

  • To extend the post-withdrawal linkage support service to other withdrawal modalities as resources allow.
