Daniel Andrews MP, Parliamentary Secretary for Health launched the publication "Community Health Services – program snapshots" on Friday 22 July 2005 at the Community Health Victoria Conference 2005.

The publication is an exciting and informative collection of sixteen snapshots about people across Victoria whose health and wellbeing has improved significantly because of the services they have received from their local Community Health Service.

"Community Health Services – program snapshots" is an addendum to the policy "Community Health Services – creating a healthier Victoria". The snapshots demonstrate the capacity of Community Health Services to be a platform for delivering a comprehensive range of primary health care. They also showcase best practice in each of the other strategic directions highlighted in the policy: community-based disease management and ambulatory care, expanded primary medical care, focus on child and family health and leadership in health promotion.
