Improving Chronic Disease Care: Lessons from the Integrated Disease Management ProjectsPage content: Overview | Contact | Download document OverviewIntegrated Disease Management (IDM) is a comprehensive and multidisciplinary approach to the care of people with, or at risk of, a particular disease or condition, aiming to reduce the burden of disease through a holistic approach. It encompasses the continuum of care from prevention through to treatment, management and maintenance. Studies strongly suggest that people with chronic disease have a better quality of life, experience fewer complications and reduce their overall use of health care resources if they participate in an IDM program tailored to their needs. Following an evidence based review, five IDM pilot projects were funded for three years in 2001 under the Department of Human Services (Victoria) IDM Initiative. ContactAdele Hamlyn Download document
