Consumer Pathway through Service CoordinationA flowchart of a consumer's pathway through service coordination is available. This flowchart identifies the activities undertaken throughout this process, and includes the use of tools to assess urgency and level of risk. The Priority Tools included in the Towards a Demand Management Framework can be utilised for this purpose.
