Overview

Care planning forms an essential element of service coordination, as articulated in the Victorian Service Coordination Practice Manual.

The Medicare Benefits Schedule funds a wide range of care planning and case conferencing services through the Enhanced Primary Care Chronic Disease Management (EPC-CDM) Program. These services may be led by a GP, or involve a contribution by a GP, and may involve the community health sector in a partnership approach to care.
