Overview

Community Health Services provide a platform to lead and deliver integrated health promotion and prevention programs targeting risk factors and promoting protective factors for people with, or at risk of, Ambulatory Care Sensitive Conditions (ACSCs). This is articulated in Community Health Services – Creating a Healthier Victoria and the DHS Integrated Health Promotion Resource Kit.

The Medicare Benefits Schedule funds services that help prevent the onset of chronic and vaccine-preventable diseases. Medicare business rules stipulate that these services must be coordinated by the client’s GP in order for the item to be claimable, though much of the work can be delegated to an appropriately trained nurse or Aboriginal Health Worker and may involve the community health sector in a partnership approach to care. Note that only certain types of client are eligible for MBS-rebateable preventive services.
