Overview
Assessment forms an essential element of Service Coordination, as articulated in the Victorian Service Coordination Practice Manual.
The Medicare Benefits Schedule funds services that assist in the assessment of a client’s medical, physical, psychological and social function through the Enhanced Primary Care (EPC) Program. Medicare business rules stipulate that these services must be coordinated by the client’s GP in order for the item to be claimable though much of the work can be delegated to an appropriately trained nurse or Aboriginal Health Worker, and may involve the community health sector in a partnership approach to care. Note that only certain types of client are eligible for MBS-rebateable health assessments.
