Overview
Care Planning forms an essential element of Service Coordination, as articulated in the Victorian Service Coordination Practice Manual.
The Medicare Benefits Schedule funds a wide range of care planning and case conferencing services through the Enhanced Primary Care Chronic Disease Management (EPC-CDM) Program. These services may be led by a GP, or involve a contribution by a GP, and may involve the community health sector in a partnership approach to care. There is also MBS funding for practice nurses to monitor client progress against a GP-led care plan. Note that only certain types of client are eligible for MBS-rebateable care planning and case conferencing.
