to enhance information management including recall and reminder systems and

Service Coordination
Service coordination aims to place consumers at the centre of service delivery
- ensuring that they have access to the services they need, opportunities
for early intervention and health promotion and improved health outcomes.
Service coordination is facilitated by Primary Care Partnerships where agencies
come together to agree on how they will coordinate their services so that
consumers experience a health system that works together. Information on
PCP Service Coordination reforms are available from the Primary
Care Partnerships website.
More than 400 GPs and general practice staff have been working with other
primary health care providers to improve service co-ordination for share
clients. This work is leading to improved quality of services; through less
duplication; improved privacy practice; better care co-ordination and improvements
in efficiencies and agency practice.
A range of Department of Human Services
program areas and their service sectors have now implemented service coordination
and a number of other Programs will be mandating the use of the Service Coordination
Tool Templates by 2006. More detail about Service Coordination implementation,
