A resource kit developed as part of the Better Healthcare in Gippsland (BHCiG) project, a joint Commonwealth/State funded initiative. One focus of this work included the regional implementation of the Wagner Chronic Care Model.

- Improving Chronic Disease Care: Learnings from the Integrated Disease Management Projects
Integrated Disease Management (IDM) is a comprehensive and multidisciplinary approach to the care of people with, or at risk of, a particular disease or condition, aiming to reduce the burden of disease through a holistic approach. Five IDM pilot projects were funded for three years in 2001 under the Department of Human Services (Victoria) IDM Initiative.

- Wagner Chronic Care Model
The Primary Health Branch has endorsed the Wagner Chronic Care Model as the framework to develop a service system that responds to the needs of clients with chronic and complex care needs. This site provides information concerning the six fundamental areas identified by Model that encourages high-quality chronic disease management across a variety of chronic illnesses, health care settings, and target populations.
- Wagner Chronic Care Model Toolkit
This online toolkit provides step-by-step descriptions of the specific changes involved in Chronic Care Model implementation, including more than 60 tools created by leading organisations for quality improvement initiatives, strategies to address financial and operational barriers to quality improvement and stories from organisations that have undertaken successful quality improvements and service system developments to improve chronic care.
