Chronic disease managementApproximately 70 per cent of the total burden of disease in Australia is attributable to six disease groups, all of which have the potential to be either prevented or managed in settings and modes other than bed-based hospital care. Type 2 diabetes is included amongst these. It is the second leading cause of disease burden in males and the sixth in females. However, as the disease also increases the risk of other conditions, diabetes is the top ranked cause of disease burden in both men and women. The Victorian Government has responded to the growing burden of chronic disease with a number of initiatives to prevent and manage chronic disease. These have been supported by various policy documents including ‘Community Health Services - creating a healthier Victoria’ released in September 2004. For more information see:
