Life! - Taking action on diabetes

Chronic disease management

Approximately 70 per cent of the total burden of disease in Australia is attributable to six disease groups, all of which have the potential to be either prevented or managed in settings and modes other than bed-based hospital care.

Type 2 diabetes is included amongst these. It is the second leading cause of disease burden in males and the sixth in females. However, as the disease also increases the risk of other conditions, diabetes is the top ranked cause of disease burden in both men and women.

The Victorian Government has responded to the growing burden of chronic disease with a number of initiatives to prevent and manage chronic disease. These have been supported by various policy documents including ‘Community Health Services - creating a healthier Victoria’ released in September 2004.

  • Integrated Chronic Disease Management
    Integrated Chronic Disease Management occurs when health services work together with the client (and /or their carer) with a chronic condition to improve their health and well being and quality of life. Those people with chronic disease receive responsive, person centered care over time through the different stages of disease progression.
  • Early Intervention in Chronic Disease in Community Health (EIiCD)
    This initiative provides community based early intervention services for people with chronic diseases.
  • Diabetes Self Management (DSM) program
    The Diabetes Self Management initiative uses the evidence-based self-management approach as a key component to better manage diabetes.

    This section provides information concerning the role and activities of the industry advisors in supporting PCPs in their integrated chronic disease management work.

    This section includes reference material, guidelines, templates and tools.
