Industry Advisor Integrated Chronic Disease Forum, 4 June 2008Page contents: Overview | Market Stalls | Presentations | Photographs OverviewPrimary Care Partnership (PCPs) and regional DHS staff across Victoria recently had the opportunity to share approaches to Integrated Chronic Disease Management and to identify key strategies in a very successful half day forum. A ‘market stall’ concept allowed attendees to participate in ‘stalls’ of interest where a short presentation was followed by discussion in a small group format. Following an introduction on policy context, five guest presenters from five PCPs generously shared their time, experiences and resources in their approach to Integrated Chronic Disease Management. The forum concluded with a short presentation on the statewide implementation of ICDM within PCPs and a panel discussion featuring all five presenters. The forum was attended by 35 registrants from 22 PCPs and all eight regional DHS offices. Feedback indicated that the market stalls were particularly well received and the generosity of the guest presenters greatly appreciated. Market Stalls1.
