EIiCD Target populationPeople with chronic disease (with or without complex needs) who may require hospitalisation in the medium to long term. Services providedClients can access a range of health and medical services including community health funded integrated care coordination, nursing, allied health, counselling and self-management interventions. EvaluationThe department commissioned the Australian Institute for Primary Care (AIPC) to undertake a state-wide evaluation of the EIiCD initiative, including the role of the Primary Care Partnerships (PCP). The evaluation set out to demonstrate improvements at the systems level and the resultant benefits to clients. The evaluation has now been finalised and the executive summary of the final report and the technical report in full provide details about the evaluation methodology and outcomes.
