EIiCD Community of Practice

Overview

A Community of Practice (CoP) is defined as ‘a network of members with a common interest sharing information, disseminating learnings and problem solving as a group’.

The Early Intervention in Chronic Disease CoP aims to:

  • Share Learning – particularly around implementation of the guidelines and service system redesign
  • Identify and discuss common issues and potential solutions
  • Promote best practice
  • Provide opportunities for communication between services and peer support

It is intended that the agenda will complement existing workforce development activities that key stakeholders are involved in rather than duplicate.

Wednesday 22 July 2009 - 'Setting the Scene and Dispelling the Myths'

This successful forum brought together representatives from both Early Intervention in Chronic Disease (EIiCD) programs and Hospital Admission Risk Program (HARP) to explore issues of mutual interest.

Friday 17 April 2009 - 'Change Management and Lean Thinking in the Health Environment'.  

 'Get going and then get better!'  - Facilitated by David Smith from the Australian Continuous Improvement Group, the day focused on the practical application of the lean thinking approach in the health environment, more specifically in the development of the EIiCD initiative and ICDM. A case study from the Peter James Centre illustrated the cultural change arising from their use of the ‘Lean Thinking’ approach and the resulting benefits.

Thursday 30 October 2008 - The use of data to support practice improvement and evaluation

The benefits of using data to make decisions and guide the continuous improvement process required to reorientate the service system to support people with chronic disease were highlighted in presentations from three EIiCD services. A presentation by Anne Markiewizc illustrated the use of program logic to determine evaluation and monitoring requirements.
