Project Scope

Following extensive local, national and international research, the scope of the project was initially contained to clinical incidents, but in August 2007 was broadened to include Consumer Feedback (including complaints, compliments and enquiries) and Occupational Health and Safety incidents.

Incident Types – In Scope

  • Clinical incidents – Events or circumstances which could have, or did lead to unintended and/or unnecessary harm to a person receiving care. This includes adverse events and near misses.
  • Consumer feedback (including complaints, compliments and enquiries)
  • Occupational health and safety incidents (staff incidents)
  • Incidents of all severity levels, including near misses (incidents that don’t cause harm).

Health Services – In Scope

All publicly funded health services:

  • 88 Public Health Services (and all services auspiced from these services)
  • 39 Funded (stand alone) Community Health Services
  • Ambulance Victoria (formerly known as Metropolitan Ambulance Service, Rural Ambulance Victoria and Alexandra District Ambulance Service
  • Royal District Nursing Service and Ballarat District Nursing and Healthcare
  • 14 Bush Nursing Centres
  • Forensicare
  • 5 Incorporated Residential Aged Care Services

Health Services – Out of Scope

The following services are currently excluded from the VHIMS scope:

  • Private health services
  • Non-government organisations
