Root cause analysis (RCA) training
The RCA training provides an overview of the RCA process in particular:
- verifying and defining the problem
- commissioning a RCA investigation
- forming the RCA team
- gathering information
- mapping and identifying critical events
- analysing and identifying root causes
- solution identification
- writing root cause statements
- developing, documenting and presenting recommendations.
Training dates
The forthcoming training dates are:
- Friday, 11 November 2011
- Wednesday, 15 February 2012
- Wednesday, 30 May 2012
- Thursday, 6 September 2012
All sessions are a one day workshop from 9am to 5pm.
RCA update (virtual meeting)
When: Thursday 29 March 2012, 1.30-3.30 pm
Duration: 2 hours
Location: The session will be held remotely using WebEx and teleconferencing facilities.
Purpose: Provide an update on the RCA process methodology.
Prerequisites: Prior completion of the Department of Health RCA facilitator training.
Registration of interest is essential to participate.
Prerequisites
In order to attend the RCA training you must have completed all three Clinical Risk Management (CRM) e-Learning packages:
- VHIMS e-Learning
- Open disclosure e-Learning
- In-depth case review e-Learning.
