The Department of Human Services Cemeteries and Crematoria Program has produced the following information to assist applicants and trusts to meet the requirements of the guidelines. There are two sets of information:
The information sheet for trusts outlines how the guidelines affect cemetery trusts, and the trust member application kit outlines the requirements for applicants. Both application kits include all of the necessary application forms and documentation. Tips for completing forms online
Incomplete applications will be returned. A checklist has been included in the 'Information kit for trusts' to assist trusts and applicants in ensuring that all forms and documentation have been completed and included for return to the department. The SSA aims to ensure that all public entities serve the Victorian community in a manner that is responsive, accountable and highly professional. The SSA can provide trusts with information and advice on many trust governance issues. The SSA website has a ‘public sector governance’ section which has information and guidelines for trusts, including a ‘good practice’ guide for new and existing trust members. Trust member resignation formIf you are an existing trust member and wish to resign, you must complete a resignation form. You can contact the Cemeteries and Crematoria Program for a form, or download here. |
