Guidelines for the appointment of trust members

Guidelines for the Appointment and Remuneration of Part-Time Non-Executive Directors of State Government Boards and Members of Statutory Boards and Advisory Committees (guidelines) have been issued.

The Department of Human Services Cemeteries and Crematoria Program has produced the following information to assist applicants and trusts to meet the requirements of the guidelines.

There are two sets of information:

  1. Small to medium sized cemetery trusts
  • Information sheet for trusts,
  • Application kit for prospective members of small to medium sized trusts,
  1. Largest fourteen cemetery trusts
  • Information sheet for the largest fourteen cemetery trusts,
  • Application kit for prospective members of the largest fourteen cemetery trusts

The information sheet for trusts outlines how the guidelines affect cemetery trusts, and the trust member application kit outlines the requirements for applicants. Both application kits include all of the necessary application forms and documentation.

This information sheet contains information for small to medium sized cemetery trusts on the following:

Trust member application kit for largest 14 cemetery trusts

This application kit contains information and application forms for applicants for membership of the largest 14 trusts on the following:

  • General note to applicants
  • Application form
  • Summary curriculum vitae
  • Declaration of private interests
  • Consent form and your privacy
  • Consent to check and release national police record (probity check form)
  • Further information

These forms can be completed on screen, printed and signed.

Tips for completing forms online

  • To fill out forms on screen, the latest version of Acrobat Reader is required (version 6.0 for PC or 5.0 for Mac). Download Acrobat Reader free from the Adobe website.
  • When completing the form, use your mouse to click into the next field - do not use the TAB key.
  • Where the same set of information is repeated within a form (e.g. name of the trust) you are only required to enter the information once. All further occurrences within the form will be automatically populated.

Incomplete applications will be returned. A checklist has been included in the 'Information kit for trusts' to assist trusts and applicants in ensuring that all forms and documentation have been completed and included for return to the department.

The State Services Authority – information for trust members on good governance

The State Services Authority (SSA) is responsible for providing advice on, and promoting, appropriate structures and governance arrangements for public entities, including cemetery trusts. The SSA aims to ensure that all public entities serve the Victorian community in a manner that is responsive, accountable and highly professional.

The SSA can provide trusts with information and advice on many trust governance issues. The SSA website has a ‘public sector governance’ section which has information and guidelines for trusts, including a ‘good practice’ guide for new and existing trust members.

Trust member resignation form

If you are an existing trust member and wish to resign, you must complete a resignation form. You can contact the Cemeteries and Crematoria Program for a form, or download here.
