Application for Addition or Modification to the VPDC Contacts List

This form should be used to add, delete or modify the VPDC Contacts database to receive information provided by the Clinical Councils Unit.

It is important that your name is on this list and that your details are up to date if you want to receive information from us relating to your work.

Having your name on this database presupposes that you are happy to receive any information relevant to your work. If you receive information that you are not happy to receive, please notify us via the VPDC Helpdesk.

Perinatal News: Clinically based updates specific to midwives
ICD Coding: Information specific to Clinical Coders and Health Information Managers regarding ICD-10-AM/ACHI coding for Perinatal data.
